How Vision Insurance works

With Unum Vision insurance, you have the flexibility to see any vision care provider and purchase materials at any location. However, you’ll save more by visiting network providers. Provider participation varies by plan, so please check yours before making an appointment.

Our vision plans typically cover eye exams, eyeglass lenses and frames, and contact lenses. Also, you’ll receive special pricing for non-covered services for additional purchases including discounts on laser vision correction (LASIK) at participating providers.

How do I submit a claim?

Many of our in-network providers offer the convenience of submitting claims on your behalf. However, if you find yourself needing to submit a claim, you can access claim forms and instructions through our online portal. Additionally, you may contact the number on the back of your ID card to request a claim form.

How do I know if my provider is in-network?

To find a new eye care provider or check if your existing provider is part of our network, select the card above that matches yours. You'll then be taken to the correct coverage portal where you can search for providers in your area.

How does Vision Insurance work?

Your vision care provider can submit a claim on your behalf to help cover your visit and the care you've received. Most policyholders choose to have benefits paid directly to the provider.
